With forms you can collect information from members of your football academy (players and parents), or build an evaluation that a coach fills in.
Steps
In the platform, select Forms from the menu.
Click Add form in the top right.
Fill in the Form title and, optionally, the Description and Deadline.
Under Who should submit it?, choose Player (filled in by the player or parent) or Coach (filled in by the coach, e.g. for an evaluation).
Add fields with the Add field button. For each field, choose a type (e.g. Long text), write the Question and, if you want, a description. Use the Required switch to set whether it must be answered.
Click Create in the top right.
What happens next
The form appears in the Forms list. To send it to players, assign it.
